Whether you’re already established or a new patient to us, getting started is easy. You can download and print our Membership Plan Application along with the Membership Agreement form and begin filling them out from home.
Once completed, you can either,

Once received, they will be reviewed and we’ll confirm availability within 1-2 days.
If all of that doesn’t sound too easy, please come by my office, take a look around and I’ll be happy to provide you with copies of everything you need to get started in person.
Once you are enrolled at Gold Country DPC, your monthly membership fee will be automatically debited from your checking or credit card account.
