Covered Services

Same day and Next day Appointments:  Monday, Tuesday, Thursday and Friday during normal business hours 8:00am to 5:00pm.

Dr. Patterson will be available by telephone or secure text messaging from 8:00am to 5:00pm, Monday thru Friday.

Unlimited office visits to provide comprehensive primary care medicine for both chronic and acute management of diseases.  Normal visits range from 15-30 minutes and you can request up to an hour.

-Physical exams (Annual Preventative/Well Child/Female GYN & Breast/Sports Physicals)

-Blood pressure/Weight checks

-Medical education (Diabetes/COPD/Heart disease)

                 -Acute Care visits, i.e. Sick visits

Procedures included in your membership:

-Minor procedures/Mole/Skin tag/Cyst removal *

-Dipstick urinalysis

-Ear Lavage/Wax removal

-Pelvic and Breast exams/Pap smears *

-Joint injections **

-Stitches/Sutures for cuts

-Destruction of pre-cancerous skin lesions

-Skin biopsies *

-Injections (Vit. B-12/Testosterone/Allergy shots) **

*Patient will be responsible for the laboratory/pathology fee for reading the sample.

**Patient will be responsible for the cost of medication (steroid, lidocaine and Toradol are included, but other injectable medications are not)

OTHER LIMITATIONS:

-Vaccine/medication administration (other than listed above, does NOT include price of medication unless otherwise stated)

-Organization and review of historic and outside medical records.

– Use of electronic medical records to help keep Patient’s preventive services up to date and track referrals, labs, imaging reports until results are available

EXCLUDED SERVICES:

-Anything not specifically listed as a Covered Service shall be a non-covered service

-Any health care services not performed on or within the premises of Gold Country Direct Primary Care, Inc., including emergency room visits, hospital stays, specialist care, imaging and labs, etc.

-Vaccines/medications not listed above (administration is included)

-Medical tests not performed in the office (actual “reading” of pap smears for example)

-Durable medical equipment (braces, splints, etc.)
